Change of address
If you change name, address or telephone number, please let our receptionists know by calling or writing to us. If you move outside the practice area you may need to find a doctor in your new area. To register with a new doctor visit the surgery of your choice, if you have your medical card take that with you.
We are a training practice whom teach medical students and sometimes you may be asked to see the doctor or nurse with a student present. You will always be asked beforehand if this is acceptable and you may ask for the student to withdraw if you prefer to have your consultation in private.
There is good access for the disabled to all of our consultation rooms. A designated parking bay is adjacent to the surgery entrance and the surgery has a ramp and wide doors to allow for wheelchair access.
We take Concerns very seriously. If you have a concern, suggestion or comment regarding the surgery or the services we offer, please contact the Practice Manager by telephone or, if you prefer, in writing. Removal From Practice List
The law allows a GP practice to remove a patient from its list under certain circumstances. A change of home address which is outside the practices designated area, when the relationship between a practice and a patient suffers irreconcilable breakdown, and is seen as the last resort when all other options have been exhausted. Removal from the list normally follows if a warning letter in the previous 12 months to the patient from the surgery does not bring about the required change in behaviour. (It may be that in very exceptional circumstances no warning will be given.)

Access to medical records
The practice is registered and complies with the Data Protection Act 2018. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. Please contact the Practice Manager for further information
We respect your right to privacy and keep all your health information confidential and secure. It is important that the NHS keeps accurate and up-to-date records about your health and treatment so that those treating you can give you the best possible care. If you require us to give any information to any third parties i.e.: wife, husband, parents etc: you must complete a consent to share medical information form. This can be found on our forms page and handed in to reception or sent by post to the surgery.
This information may be used for management and audit purposes. However, it is usually only available to, and used by, those involved in your care. You have the right to know what information we hold about you. If you would like to see your records please contact the Practice Manager.
Patient rights and responsibilities
We aim to treat our patients courteously at all times and expect our patients to treat our staff in the same way.
If you are unable to keep an appointment please let us know in plently of time.
Please inform us of your past illnesses, medication, hospital admissions and any other relevant details.
Violent or abusive behaviour
We take seriously any threatening, abusive or violent behaviour against any of our staff or patients. If a patient is violent or abusive, they will be warned to stop their behaviour. If they persist, we may exercise our right to take action to have them removed, immediately if necessary, from our list of patients.
