The Conwy West Community Navigators provide support for people in the community to improve their mental and physical well-being.

Our navigators spend time with you, getting to know all about you and your situation. They provide Social Prescribing, a non-medical support for health and social issues and offer long-term resolutions. They do all they can to make you feel that you can speak openly about what matters to you, what is important to you, and what you want to see happen to bring about change in your life.

A series of follow up visits and calls are made, as the experienced Navigators present you with possible choices of activities, programs, or services from the local community, that are suited to you. This can include just about anything from finding a lunch club to getting help to sort out a housing issue.

The Navigator project is funded by the Conwy West GPs and supported by CVSC, and covers the following areas:  Conwy, Craig-y-Don, Llandudno, Llandudno Junction, Llanfairfechan, Penrhyn Bay, Betws-y-Coed, Llanrwst, and Cerrigydrudion.
